Acknowledgment of Risk:
I, the participant, acknowledge and understand that taking part in any activity at 'The Break Room' involves inherent risks, that include but are not limited to:
Physical injury (Cuts, Sprains, Bruises, Etc.)
Emotional distress
Damage to personal property
I, the participant, acknowledge and understand that the activities at 'The Break Room' are in environments that contain potentially dangerous equipment such as, Baseball Bats, Sledgehammers, and other tools. I also understand that there will be potentially hazardous materials, such as broken glass and electronics.
Health & Safety:
I, the participant, declare that I am physically fit, and do not suffer from any medical condition, including pregnancy, that may be affected by participating in any activity at 'The Break Room'.
I, the participant, agree to wear all of the provided safety equipment at all times during any activity at 'The Break Room'. The provided safety equipment includes but is not limited to, helmets, gloves, goggles and coveralls.
I, the participant, agree to follow all instructions delivered by staff of 'The Break Room' and will adhere to all posted safety rules and regulations.
I, the participant, agree that I will not use any equipment other than for their intended purpose. I understand that if I cause any damage to structural walls, integral shelving, furniture, or property, 'The Break Room' reserves the right to charge a £200 fee and end my activity immediately.
I, the participant, acknowledge and understand that if I am under the influence of alcohol or drugs when I arrive for my activity, then the activity will be cancelled and NO refund or reschedule will be offered.
Participant Age & Supervision:
ALL Participants MUST be aged 12 years or over to partake in any activity at 'The Break Room'
ALL Participants under the age of 18 MUST be accompanied by an adult who remains in the building at ALL times during the activity.
Release of Liability:
In consideration of being allowed to participate in any activities offered at 'The Break Room', I hearby release, waive, discharge, and covenant not to sue 'The Break Room', its owners, officers, employees, agents, and contractors from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my property during any activity at 'The Break Room'.
Indemnification:
I, the participant, agree to indemnify and hold harmless 'The Break Room' from any loss, liability, damage, or costs, including court costs and attorney fees, that may occur during my participation of any activity at 'The Break Room', whether caused due to negligence by 'The Break Room' or otherwise.
Media Release:
The Break Room uses CCTV cameras throughout the facility for security and safety purposes. By entering the premises and participating in activities, participants consent to being recorded for security purposes. These recordings will not be used for promotional purposes unless explicitly consented to by the Participant in a separate agreement.
Acknowledgement of Understanding.
I, the participant, have read this waiver of liability, assumption of risk, and indemnity agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing this agreement freely and voluntarily and intend by my confirmation of this form and completing a booking to be a complete and unconditional release of liability to the greatest extent allowed by law.
